Step 1 - Contact us!

Fill out our New Client Interest Form.


Step 2 - Setting up your Client Portal

Our EHR, Simple Practice™, will send you an email with a link to sign into your new Client Portal.

Step 3 - New Client Paperwork

You’ll receive an email with a link to complete all the online forms you need to get started as a new client.

This is done securely through Simple Practice™’s Client Portal.

Once all of the forms are submitted in Simple Practice™, our staff will process and verify your information.

Step 4 - Scheduling

Our team will reach out to you via phone and email to schedule a time for you to come in for your first appointment!
